Empower Speech Therapy is a healthcare provider. We protect your protected health information (PHI) as required by HIPAA and other applicable law.
How we may use or disclose PHI
We may use or disclose PHI for treatment, payment, and healthcare operations. We may also disclose information when permitted or required by law. We limit use and disclosure to what is appropriate for these purposes.
Your rights
You may have rights to access your PHI, request amendments, ask for restrictions, and receive information about certain disclosures. You may also file a complaint without retaliation.
Our duties
We are required to protect the privacy of your PHI, provide you with notice of our privacy practices, and follow the terms of the notice currently in effect. We may update this notice when permitted by law.
